Outlook care operates residential care homes for customers with a primary diagnosis of Learning disability, we support customers with complex care and support needs to live in Homes that are welcoming, with a staff group that can ensure their needs are fully supported.
What is residential care?
Residential care homes within Outlook care offer personal care and support to adults living with complex conditions who require specialist support to care for their daily needs , our customers who live in our residential homes cannot live independently in their own home.
Our residential homes are able to offer comfortable and homely atmospheres to our customers , they will have their own individual bedroom and there will be shared communal areas such as living room, dining room and kitchen.
Our residential care homes are suitable for people who will benefit from 24-7 specialist support. Each home is designed carefully to keep the people we support safe and comfortable whilst helping them to build new skills, we ensure that any specialist equipment is discussed with the funding authority and that customers are provided with equipment to meet their individual needs , where possible we ensure that all professionals are included in the assessment and transition process to ensure that such items are in place before admission to the home .
Our staff groups, supported by an experienced management team, work closely with our customers to develop a person-centred support plan that will allow our customers to have the best day everyday. We work closely building partnerships with other professionals and the circle of support for the customer to ensure that all of their needs are met.
Each home provides customers with the opportunity to enjoy spending time with others in the communal living spaces and kitchen areas, with the privacy of their own bedroom with en-suite wet room or bathroom facilities, we encourage our customers to personalise their bedrooms and our staff will ensure that they are fully included in choices made about their living space .
Below you will find listed our current Learning Disability residential care servies, registered with the Care Quality Commission. This includes 357A - 359 Dagenham Road which is a registered care home for mental health.
